The New Beacon app is designed for healthcare professionals to easily refer patients to our Hospice. Healthcare providers will find a General Hospice Criteria checklist, disease specific guidelines, a BMI calculator, and an easy to use eligibility checklist. The APP will help locate and contact a hospice location convenient to your patients and their families.
